Head Injury

Data gathering

  • Red flags – LOC, drowsy, progressive headache, recurrent vomiting, amnesia, warfarin, CSF leak
  • Others – mechanism
  • Examine – CN, bruising around eyes/ears

 

Clinical management

  • Investigations
    • CT head
  • Treatment
    • Analgesia
    • Supervision 24hours
    • Avoid driving, alcohol, sedatives until fully recovered
  • Followup / Safety-net
    • concussion, written info – headaches, reduced concentration
    • ask friend/family to read information
    • review in 2 weeks if ongoing

NB:

  • Beware of delayed subdural particularly in elderly, alcohol excess, confusion, balance changes, increased falls
